News

January 17, 2013

Exponent engineers Ali Reza, P.E., CFI, and Zuhair Ibrahim, Ph.D., P.E., assisted the US Chemical Safety Board (CSB) with their investigation into a fatal explosion at a fireworks storage bunker in Oahu, Hawaii in April 2011. Exponent assisted with the origin and cause investigation of the explosion, and conducted a codes and industry practices review regarding guidelines and standards addressing the safe procedures for dismantling and destruction of fireworks.

The CSB issued several recommendations, including a suggestion that industry best practices or standards regarding safe disposal of fireworks should be created. The Exponent report was cited in the CSB investigation report and Mr. Reza, from Exponent’s Thermal Sciences practice, was featured in their concurrently released video “Deadly Contract”.

The CSB final investigation report and the video may be accessed at the CSB website or on YouTube.
